Payment Options
We are preferred providers for most private health plans. As a courtesy to our patients, we check your insurance benefits to be sure that you have current physical therapy coverage. We also work with the insurance companies to obtain authorization for services when it is required.
We offer insurance billing services to our patients through BCM Services, a local billing and collections agency. Co-payments are collected at the time of service. Patient balances will be billed on a monthly basis for any balance owed after we have received payment from the insurance company (such as coinsurance and/or deductibles).
Auto Insurance
Any patient involved in a motor vehicle accident may choose to have their auto insurance billed using their "Med Pay" benefits, if included in their policy. Mountain Valley Physical Therapy will verify with the auto carrier that there is a "Med Pay" policy and that there are still funds available. Any balance not covered by the "Med Pay" policy of a patient will be billed to the patient on a monthly basis and is the patient's responsibility to pay.
Workers' Compensation
Injured workers have 100% coverage for accepted claims through their employer's workers' compensation insurance. Prior authorization is usually required by workers' compensation insurance companies before physical therapy treatment may begin. We will work with you and the insurance company to obtain authorization in the shortest time frame possible.
Self-Pay
Patients who do not have insurance coverage or wish to have wellness visits without utilizing their insurance may do so under the Colorado Direct Access policy. We offer affordable and competitive self-pay prices for our services as well as package deals.
Accepted forms of payment
We accept payment for services by cash, check and major credit cards including Visa and Mastercard.
Please contact our office for physical and massage therapy rates, as well as package deals available for massage and stretching.
